Beginning of the Year Health Requirements
Dear WCS Families,
Parents and guardians of students entering preschool, kindergarten, second grade, and sixth grade must submit required health forms – or proof of an upcoming appointment – to their school nurse or to health.forms@d300.org by Friday, August 1. If your student is in any other grade and has up-to-date physical and immunization records, no additional forms are needed. Please note that those who are not up-to-date with the health exam and immunization requirements by the first day of attendance on Tuesday, August 12, will not be allowed to attend school until all required forms or proof of upcoming appointments are submitted.
Additional health requirements:
- All students entering kindergarten, second, and sixth grades are required to have a dental examination
- Dental examinations are due by May 15th of the year the student is enrolled in kindergarten, second, sixth, or ninth grade. A current examination is one that was completed within 18 months of May 15th
- All students enrolling in a kindergarten program or enrolling for the first time in a public school shall submit proof of an eye examination completed within the previous year before October 15th of the school year
