Incoming 6th Grade Physicals

All incoming 6th grade students MUST have a current physical and TDAP and Meningitis (MCV 4) immunizations completed and on file in the middle school nurse's office by August 1st.  Any student who doesn't have their physicals and immunizations completed and received will be

excluded from school on the first day. If your student currently has a physical dated 08/01/2025 or later. Please email them or drop off to the middle school office now. They will satisfy the requirement for the school year. TDAP and Meningitis

vaccinations are given after age 11.  If your student's appointment is occurring after the first day of school, you MUST email the nurse's office with the date and time of their upcoming appointment.  Please email physicals to Debra.Peterson@D300.org. which is the nurse's
